Insurance subsidy

Claim the fixed subsidy (Festzuschuss) from your health insurer

Even for treatment elsewhere in the EU, you are entitled to the fixed subsidy (Festzuschuss) from your statutory health insurer (gesetzliche Krankenkasse). Important: your treatment-and-cost plan (Heil- und Kostenplan, HKP) must be approved in writing by your insurer before treatment begins. This pre-approval (Vorabgenehmigung) is mandatory for treatment abroad in the EU. We make sure your plan is ready in good time and in the right form, so your subsidy is not lost.

1. The Heil- und Kostenplan on your insurer's form

First request the Heil- und Kostenplan form from your statutory health insurer. The clinic in Sofia completes your cross-border treatment-and-cost plan on it in German, with the written note that the work is carried out elsewhere in the EU.

2. Get it approved before you travel

Submit the Heil- und Kostenplan together with your bonus booklet (Bonusheft) to your insurer and wait for written approval before you travel to Sofia. This Vorabgenehmigung is mandatory. Processing usually takes around 3 weeks, or up to 6 weeks if an expert review is needed. The approved plan is valid for 6 months.

3. Treatment, then reimbursement

You pay the clinic and then submit the paid invoice to your insurer. You are reimbursed the approved fixed subsidy, capped at the amount your insurer would also have covered for the same treatment in Germany.

We make sure the clinic in Sofia issues a correct, German-language cross-border Heil- und Kostenplan that your statutory insurer accepts, and we schedule your dates so the plan is approved before treatment begins. Legal basis: § 13 (4) to (5) SGB V and the EU Patient Mobility Directive 2011/24/EU, with the fixed subsidy under § 55 SGB V. Privately insured patients (PKV) are reimbursed according to their own policy.

The size of the fixed subsidy is based on the standard care (Regelversorgung) and your bonus booklet (60% base, 70% with a complete 5-year Bonusheft, 75% at 10 years, and up to 100% of the standard care in hardship cases). For implants, the insurer subsidises only the crown or bridge on the implant, not the surgical procedure itself.
